2010/2011 Season Club Sign-on

The Albany Creek Swim Club 2010/2011 season sign-on day was held at the Albany Creek Leisure Centre on
Saturday, 21-Aug-2010 between 7:00am and 11:00am.  BUT...it's not too late to join..!

The fee structure for the 2010/2011 season is as follows:

• One child = $145
• Two children = $290
• Three children = $435
• Fourth child is free

Club uniform orders can also be made at this time (a separate cheque is required for uniform purchases).  Order forms can be found on this website at the bottom of the Club Merchandise page.

Please contact Leanne Blaney (or one of the other committee members) if you have any questions.  Please drop all completed forms into the locked wooden box at the club.  You will need to provide:

• A Membership Form for each swimmer being nominated (filled out & signed)
• A code of Conduct form for each swimmer being nominated (filled out & signed)
• A copy of each swimmers birth certificate (only if nominating new members)
• Cash or cheque (made out to Albany Creek Swim Club) for the total of all swimmers being nominated
